If you had a health crisis, how would you want decisions made about your care, and who would you want making those decisions? This workshop will cover the basics you need to know to write your own Advance Directive (AD). Once completed and shared with your health care providers, your AD will guide care during a physical health crisis and/or mental health crisis. Empower yourself with knowledge, and an AD!
